The highlight icon on your profile is a tiny circle, roughly 56-62 dp across. The full 1080 × 1920 image is never shown; Instagram crops to the center square and applies a circular mask. Keep your icon dead center.
Why 9:16?
Highlight covers are stored internally as Story frames. Instagram expects 1080 × 1920 and crops from the center for the circle icon. Uploading a different ratio forces Instagram to auto-crop, which may shift your icon off-center.
